CHICAGO, ILL. (April 26, 2021) — This community’s response to joining Telligen to address high priority health issues during COVID-19 and beyond has demonstrated a high level of commitment towards the overall health of your community. The pandemic has not only highlighted the importance of community members working together, but also how collaboration can drive community-centered health outcome improvement.
For more than four decades, Telligen has assisted multi-sector partners with improving community-wide health and wellness. We’re honoring your hard work, passion and dedication to providing patients, families, neighbors, and staff with the highest quality of care. We know that our partners are busy doing the important work of caring for your people. Having the time to bring everyone together, define, and organize collective action is extremely complex and challenging not only now amidst COVID, but always. That is what we are called, and expected, to do and we are held accountable for our performance. The tools we use are rooted in evidence, action, measurement, timelines, and CELEBRATION towards achievement!

The local communities in each of our four states – one rural, one urban, and one mixed – with the most organizations that have joined Telligen QI Connect™ have been named “Community Quality Champions”. Telligen is pleased to present this award in recognition of outstanding activation by organizations located in your home community. Recipients of this inaugural Telligen QI Connect™ award utilized their relationships to recruit individuals and partner organizations to join together to focus on the most challenging issues facing us all today in this time of COVID-19 and beyond: opioid misuse, increasing access to behavioral health services, ensuring smooth patient transitions with changes in level of care, patient safety, chronic disease prevention and self-care, and nursing home quality.
We also invite Medicare beneficiaries, family caregivers, and advocates who live in Telligen’s four state region (Colorado, Illinois, Iowa, and Oklahoma) to volunteer as Community Ambassadors to share what matters most to you. Community Ambassadors will participate alongside healthcare professionals, community organizations, community leaders, and other Ambassadors in our Telligen QI ConnectTM events to directly influence how care is delivered in our communities. Whether we are providing care or receiving it, all of us are needed in collective action to make healthcare safer, more accessible, and more cost-effective.
