Residents may choose from a variety of floor plans for studio and one-bedroom apartments with an extensive range of amenities. Our modern, spacious apartments have individually controlled heat and air conditioning. If you need a little more help down the line, we understand. There is no need to move to different part of our building. Our Assisted Living services come to you in your apartment.

Transitional Care Respite
$295 daily rate | Maximum 2 week/14-day stay

For individuals that need temporary skilled nursing care that are coming from home or another community. Provides caregivers a break with a temporary solution for those in need of skilled nursing care. Regular daily rates apply after 14 days.

Short-Term Therapy Respite
$325 daily rate | Maximum 3 week/21-day stay

For hospital patients who have been discharged home and Medicare does not cover short-term rehabilitation and cannot manage at home. Access to therapy through other insurance benefits or private pay. Regular daily rates apply after 21 days. (Patients being discharged from short-term therapy from another skilled facility and wish to continue therapy may also be accepted)

Extended Stay Short-Term Therapy Respite
$325 daily rate | Maximum 3 week/21-day stay

Extend your stay at Selfhelp when you’re not quite ready to go home. An option to continue your short-term therapy at Selfhelp after you’ve been discharged and insurance coverage for therapy has ended. Access to therapy through other insurance benefits or private pay. Regular daily rates apply after 21 days.

Application Process
  • Prospective residents are asked to complete an application including financial application and medical evaluation.
  • A personal interview with facility staff assures that each resident is comfortable with our facility and that we can provide the appropriate services and meet each individual’s needs.
