Social Services Assistant, Full Time
Summary/Objective
In keeping with our organization’s goal of improving the lives of the Residents we serve, the Social Services Assistant ensures that the medically related emotional and social needs of the Residents are met and maintained on an individual basis and in accordance with current federal, state, and local regulations. Skilled nursing facility social services experience preferred.
Salary Range:
The position is full-time.
$25.00-30.00 per hour depending on experience.

Job Description:
1. Responsible for keeping up-to-date evaluation documentation on each Resident’s cognition, mood and behavior which complies with Federal, State, and Local regulations.
2. Completes upon admission a comprehensive social history and psychosocial assessment that includes the Resident’s problems and strengths and preferences, including cultural, religious, and ethnic background and implications for the care plan.
3. Discusses and Educates Residents and families on advance directives on an ongoing basis and updates the Interdisciplinary Team of changes to advance directives.
4. Documents all interactions and communications with residents and families as it relates to the resident’s cognition, mood, behavior and/or well-being
5. Orients Residents and families to the Residents’ Rights, the Colbert and Williams Program and facility services.
6. Completes scheduled MDS assessments with corresponding documentation within the scheduled assessment period.
7. Creates, updates and maintains Residents’ individualized care plans in conjunction with the corresponding MDS assessment and as needed.
8. Attends scheduled Interdisciplinary Team care conferences and identifies and interprets the psychosocial needs of Residents that reinforce individuality and identity for inclusion in the care plan.
9. Discusses the facility rules and regulations with Residents’ families and responds to questions regarding these rules.
10. Provides 1:1 counseling for residents who self-isolate and demonstrate on-going behaviors.
11. Completes mandatory documentation within timelines established by applicable regulations.
12. Provides consultation to Residents on a regular or as needed basis to discuss problems encountered by Residents and assists with the implementation of solutions.
13. Addresses Residents and family concerns, document on concern form and file in the concerns binder.
14. Mediates issues that arise among Residents, families and staff.
15. Provides informal counseling when needed to uncover any problems which might interfere with the Residents’ socialization.
16. Informs nursing and the Social Services Director of observed changes in the resident’s mood, behavior and/or well-being.
17. Reports abuse and neglect immediately.
18. Establishes, develops, maintains and updates filing system for the Social Services Department.
19. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. Follows established safety precautions when performing tasks and using equipment and supplies.
20. Maintains the comfort, privacy and dignity of Residents and interacts with them in a manner that displays warmth, respect and promotes a caring environment.
21. Communicates and interacts effectively and tactfully with Residents, visitors, families, peers and supervisors.
22. Attend and participate in departmental meetings and in-services as directed.
23. Communicate with other employees in a private and respectful manner.
24. Work and be proficient with certain computer systems designated by the company.

Work Environment
This job operates in a health care setting. This role requires regular walking to various locations around the facility. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
This role may also come into contact with Residents who may have contagious illnesses.
