Short-Term Rehabilitation

Although our process looks different, our commitment to guiding and motivating you to heal and regain your abilities remains the same. Our goal is to work with you and your family to achieve your rehabilitation goals and make the transition from rehab to home simple and smooth and… ensuring you have a positive experience along the way.
Tours
Residents and their families are welcome to come to The Selfhelp Home in person to tour our rehab unit and community. A scheduled tour in advance is required. You can request a tour here. Negative COVID-19 Test Prior to Admission
A negative COVID-19 test is required within three days of admission from a hospital.
Quarantine
A 14-day quarantine is ONLY required for unvaccinated individuals arriving to The Selfhelp Home.
Therapy Services
Therapy will be conducted in our therapy gym unless a 14 -day precautionary isolation period is required. In this case, therapy will take place in the resident’s room for the first 14 days. We will limit the number of people in the gym at one time to ensure proper physical distancing. Care Plan Meetings
Care plan meetings can be arranged with our care team members by phone call or in-person to review treatment plans and rehabilitation goals.
Private Caregiver
Individuals coming to Selfhelp for short-term rehabilitation may have a private caregiver. The private caregivers must have a COVID-19 test three days prior to admission. Private caregivers must be properly registered with The Selfhelp Home and must be up-to-date with their COVID vaccine shots.
